Community Health Needs Assessment Survey Now Open for Participation

Mile Bluff Medical Center is conducting its Community Health Needs Assessment (CHNA) now through November 30. It is an anonymous survey aimed at identifying key health issues that impact residents in South-Central Wisconsin. The survey takes just 10-15 minutes to complete; and is open to those 18 and over.
The CHNA is a vital step in a multi-year process that helps the medical center align its efforts with the evolving needs of the community. The insights gathered guide the development of initiatives tailored to address specific challenges faced by residents. Past assessments have highlighted community strengths such as a clean environment and access to affordable healthcare, while also identifying areas for improvement, including job availability and access to affordable housing and mental health services.
Residents are encouraged to share their experiences and opinions through the survey. There are questions about physical and mental health status, access to community resources, demographics and more. The feedback received will be used to shape community health initiatives over the next three years.

- Scan the QR code on a flyer you see in the community.
- If you don’t have access to Internet, pick up a paper survey at the main desk of any Mile Bluff facility.
