Medical Records

Mile Bluff Medical Center is committed to protecting your privacy and confidentiality. Before we release copies of your medical records, we require patient or legal guardian authorization. The Health Information Department maintains all Mile Bluff patient healthcare records and reviews all requests for copies before processing.

Requested records are sent by mail. The original records are kept at Mile Bluff Medical Center. You may request copies of your records at any time.

You can request your own medical records online, or complete and return a paper form.

Use the Authorization for Disclosure of Protected Health Information form if you:

• prefer to use a paper form for your request
• want your records to be shared with another organization
• are requesting records that contain information about HIV/AIDS, mental/behavioral health conditions, or alcohol/drug abuse treatment

You may also request a form by calling the Health Information Department at 608-847-1855 to request a form. Some copying fees may apply.

If you are looking to become a new patient at Mile Bluff, please call one of our clinics to make an appointment. Our staff will provide you with information on becoming a new patient at Mile Bluff as well as steps you can take to request your records from your previous provider.

An attestation form must be completed by entities requesting records that are potentially related to reproductive health care. Send completed forms to our Health Information Department using the fax number or mailing address above.
