Your First Clinic Visit

area families have trusted their care to us. We are glad that you do too.
As with any new relationship, it takes time to get to know each other.
Your initial clinic visit is a great opportunity for us to learn about
you, your health, and your preferences. Here are a few tips to help ensure
that you have a successful first visit.
What to bring:
- Photo identification
- Insurance information
- All medications (prescriptions, over-the-counter, supplements, oils, creams,
drops etc.) in bottles with dosage and frequency information
We request that you:
- Arrive 30 minutes early for your first appointment - This allows us time
to get to know you, and to ensure that we have all the information we
need for a great visit.
- Bring a list of your medical questions and concerns - This will help us
to prioritize your concerns so that we cover the most important issues
in your first visit. We can schedule a follow-up appointment, if needed.

Fill out a
health history form - By completing this form before your appointment, you’re less likely
to forget important information. If you do not get a chance to fill out
the form ahead of time, we will have one for you to complete at the office.

Complete a
medical records release form
Sometimes life happens. If you cannot make your original appointment, please
inform us at least one business day in advance. We will find another appointment
time that works for you.
